Instructions for Using the Self Service Form
Preparation Program

Welcome to the Self Service Form Preparation Program. This online program can help you fill out court forms used to file a lawsuit.  This program does not give legal advice.

You will be asked a set of questions relating to your case.  Please answer these questions as completely as possible. The information that you provide to us will populate into the appropriate court forms.  This program does not automatically correct your grammar, spelling or punctuation.

Using your user name and password, you will be able to view, edit and print your documents for 7 days.  You may use the "PREV" and "NEXT" arrows, as well as the menu on the left, to return to and edit information you've already entered.

If you come across a legal term or word that you don't know, you may get the definition by placing your mouse over the words that are highlighted in blue.  You may also access a glossary of legal terms by clicking the GLOSSARY link.
